Changing the name on your passport before your ceremony

A PD2 form allows you to change your name on your passport before your marriage or civil partnership ceremony.

  • We can only complete PD2 forms for ceremonies taking place within our district.
  • Your ceremony must take place within 3 months of the form being completed.
  • Your new passport will be post-dated and will only be valid after your ceremony has taken place.
