Personalised care represents the relationship between you, professionals and the health and care system. We support primary care to deliver non-medical support to patients across all the primary care networks in Kirklees.

Personalised care means you have choice and control over the way your support is planned and delivered. It is based on what matters to you and your individual strengths and needs.

How it benefits you

Services are focussed on you including health, social care, public health and wider services. It provides an all-age approach from maternity and childhood right through to end of life. It encompasses both mental and physical health. It recognises the role and voice of carers, the contribution of communities and the voluntary and community sector to support you and build resilience.

What social prescribing is

Many things can affect our health and well-being which could include feeling isolated, lonely or stressed by work money and housing problems or living with a health condition.

Social prescribing helps people to improve their health, wellbeing and social welfare by connecting them to services and activities in their local community. This could include:

  • social activities
  • walking, well-being and fitness groups
  • parent support groups
  • arts, gardening and creative activities
  • employment and training support
  • housing and benefits support

Who social prescribing is for

Social Prescribing is for anyone who would like to:

  • make positive changes to their life
  • improve social connections
  • feel more connected to the local community
  • get information and advice about housing, benefits, employment or training
  • live better with a health condition
  • improve health and well-being

How social prescribing can work for you

Our social prescribing link workers will talk with you about what matters to you. They will help you to identify any issues and challenges that you would like to address and help connect you to services and local community activities that can support you.

To speak to a link worker, please contact your GP.

You must be registered with a GP practice based in Kirklees.

Health and wellbeing coaching

Health and wellbeing coaches will help you to take the steps to improve how you manage your physical and mental health based on what matters to you. You can access support on a one-to-one or group basis and can also get help to set up peer-to-peer groups.

What support is available

We can help you with:

  • Managing long term health conditions
  • Weight management and healthy eating
  • Support around managing your mental health
  • Stopping smoking
  • Reducing drinking alcohol
  • Increasing your physical activity

Care coordination

Our care co-ordinators work in the community, visiting patients at home.

They work with patients who will benefit from support to improve their health and wellbeing and achieve their self-care goals.

What support is available

Care co-ordinators work with patients to identify appropriate organisations and services that are available to help them achieve their goals. They will make onward referrals and organise a team of support to help patients better manage their own care and needs.

How to access support

Care co-ordinators will work directly with GP practices to identify patients who can access support.

Slaithwaite Health and Wellbeing Centre

Learn more about our health and wellbeing centre, opened in collaboration with The Valleys PCN, that provides more local support for residents.

We offer a range of activities to improve the health and wellbeing of local residents. Come along to one of the sessions below.

Skelmanthorpe Community Garden

Access support from our social prescribing team, meet new people, improve your wellbeing and learn some gardening skills in our upcoming community garden. Drop-ins are every Saturday 10:30am to 12:30pm and everyone is welcome.
