Community Enablement Team

Who we are

Kirklees Council's community enablement team is a service for young people and adults with learning disabilities (including autistic spectrum conditions.) We are a team of skilled, experienced and qualified staff who support individuals to achieve their goals. We are a registered provider with the Care Quality Commission

What community enablement is

  • It's all about supporting you to be as independent as you can be
  • It's person centered, we find out what you can do now and start from there
  • Exploring what you are good at and building on it
  • Choosing the things that you want to learn and how quickly you want to learn them
  • Learning the skills you need to be as independent as possible
  • Working together to solve problems
  • Helping you to build up the friendships and relationships you need to be part of the community
  • Working with the people that are important to you, like your family, friends and carers
  • Finding ways that you can do the things that you want to do whilst keeping you safe
  • Helping you to develop your confidence
  • Checking that our plan is working

Who community enablement is for

Community enablement is for everyone with a learning disability (including autistic spectrum conditions), you could be:

Criteria for a community enablement referral

  • You must have a learning disability, although this does not need to be diagnosed
  • You must have the potential and want to become as independent as you can be

After you have applied

Initial visit

One of our management team will arrange to visit you, usually at your home, to discuss your referral, explain the enablement journey and make a professional assessment whether community enablement is the best service for you

Enablement journey

If community enablement is right for you:

  • We will carry out a detailed assessment using an outcomes star. This will take about 4 to 6 weeks
  • You will choose which areas of the outcomes star you want to work on and this will be the start of you personal enablement plan.
  • Each session will have a plan and structure, you will be encouraged to take responsibility for the sessions.
  • We will monitor your progress and review your plans regularly so that we can update and change them if needed.

If community enablement is not suitable for you

During the initial visit it may become clear that community enablement is not the best service for you. If this is the case we will offer information and advice. We have well established partnerships with other agencies and departments throughout Kirklees and we will aim to put you in touch with the most suitable provider.
