Complaints and compliments form for children and young people

If you are unhappy with the way you are being looked after, or you think you are being treated unfairly in some way you might want to make a complaint.

On the other hand, if you are really happy with the way you are treated and want to let people know, you might want to make a compliment.

If you don't feel that you could talk to someone you know about this you could contact the complaints and compliments manager. You can do this by filling in the form below.
