Young people's support referrals

This page gives a brief explanation about each of the youth support programmes on offer, in addition to guidance notes on completing the form.

The Youth Inclusion and Support Panels (YISPs) are delivered through Kirklees Youth Offending Team, the Positive Activities for young People (PAYP) programmes are delivered through Kirklees Young People's Service.

Please note:

  • The form is for access to various support programmes and an assessment will take place to ultimately determine which is the most appropriate one.
  • When submitting the referral form, a copy will be sent to the email address you enter into Section 1. This email address should be that of the person making the referral. Please also ensure you check the correct typing of the email address.

When you have read through the information below, you can begin filling in the Young people's support referral form.

Information Sharing:

The referrals process will involve sharing information with a number of partner organisations, this information will be held securely and may be used in order to support the process of allocation and engagement.

By submitting this referral form a referring officer agrees to remain a point of contact and will be contacted on a regular basis to ensure the young person is supported effectively and to share any concerns.

Before you fill in the Young people's support referral form

There are some key points to remember;


  • All programmes, except the Youth Inclusion Support Panel (YISP), are based on voluntary association.
  • The young person must, and the parent/guardian (if appropriate) should, be aware of and consent to the referral.
  • The Positive Activities for Young People (PAYP) programme is currently in high demand and there are limited places. Priority will go to the young people meeting the criteria for one to one support (see guidance notes on section 6), in some cases we may not be able to offer support.
  • A completed referral will not necessarily lead to the allocation of a Key Worker on the PAYP programme.
  • All referrals will be allocated to workers in the locality where the young person lives.

Information about the programmes

Positive Activities for Young People (PAYP) Key Workers

Work with young people aged 8 to 19 years offering opportunities to get involved in various activities alongside offering information, advice and guidance according to need. The programme is focussed on reducing the likelihood of young people getting involved in offending and anti social behaviour and to improve behaviour and attendance in school.


Youth Inclusion and Support Panels (YISPs)

Multi agency planning groups that seek to prevent offending and anti-social behaviour by offering support services to high risk 8 to 19 year olds and their families. A panel of different agencies and organisations will meet to discuss each referral and a decision will be made as to whether it is suitable for YISP involvement.


If the relevant criteria has been met i.e. involved in anti-social or criminal activities up to and including receiving a police reprimand, then a YISP worker will lead and co-ordinate services based on identified needs gained from an assessment.

Guidance on completing the form

Mandatory fields

In order to complete the form, there are various sections which are mandatory - these are marked with a red asterisk. This information is required in order to correctly allocate the referral and for the relevant safety checks to be completed, the form will not be submitted unless they are completed. Where there are no red asterisks and you do not know the information, these parts can be left blank.


Section 1

In section 1, please give as much contact information about yourself as referring officer in order for us to get in touch should we need to ask you anything else, especially your email address to allow us to inform you that your referral has been received.


Section 2

In section 2, where you select the young persons employment/education status and click next, a further drop down menu will appear showing the relevant options. For example, if you select that the young person is at Secondary School, the drop down menu will give the option to select any Kirklees Secondary School.


Section 6

In section 6, we require information about the reason for your referral in order to determine the appropriate programme and level of support to offer. Please give as much information as possible about the reason you are referring the young person. For example: "John has recently been involved in problems at home with mum, leading to him being asked to leave. He is currently staying at a friend's house and needs to find accommodation. John is also currently unemployed and is trying to find work".


If you intend to make a referral for PAYP


As previously highlighted all information advice and guidance staff have limited caseloads, particularly the PAYP Key Workers due to high demand, you will note there are no longer levels of referral. If you intend to refer to PAYP, there is an option to indicate that the young person should be offered one to one support from a Key Worker on PAYP (previously level 3). Please only select this option if:


  • The young person has been known to be involved in Anti Social Behaviour.
  • The young person has been involved in offending.
  • The young person is at severe risk of being permanently excluded from school or has already been excluded from school.
  • The young person is subject to a Child In Need (Section 17) or Child Protection Plan (Section 47).

As a referring officer you much have first hand evidence of this information. For example, evidence of Anti Social Behaviour must come from an ASB officer. It may be necessary to ask a colleague in one of the appropriate agencies to support your referral with one of their own. If a young person does meet the criteria above or you cannot evidence first hand that the do, they will be sent information about other YPS provision in their local area.


Section 7

In Section 7 you should indicate at least three or more risk factors which are present with the young person and can be evidenced.


Section 9

In section 9, you are submitting the form and confirming that as a minimum, you have sought consent from the young person to the referral and to share information. If the parent/guardian has also consented, please tick the appropriate boxes. Note that you don't have to fill in all boxes in this section for the form to be submitted.
