If you are between the age of 14 and 18 and you have a “Children with a Disability Team” care manager they will talk to you about your hopes for the future.
They will help you to start thinking about the life you will have as a grown up.
They will develop something called a Pathway Plan which helps you plan your way into adult services.
If you do not have a “Children with a Disability Team” care manager, you should speak to Kirklees Gateway to care and they will put you in touch directly with adult services.
