The Single Assessment Process

Older people who need a health or social care service are able to have one overall assessment rather than being asked the same set of questions
again by different staff from different organisations.
Health and social care staff work closer together to provide single assessments for older people, which assess both their health and social
care needs. The new assessments are centred on the older person making sure they are involved in the whole process.
The Single Assessment Process is a national development from the Department of Health. It is part of the National Service Framework for Older
People. During the last year, Kirklees Health and Social Services teams have been working with older people, to introduce the process. This is
a major piece of work which will take time and careful planning - but which will see improvements as more and more agencies become familiar
with new ways of working and sharing information.
In January 2004, the first teams to begin the process included Community Social Services Teams, District Nursing, Occupational Therapy and the
Intermediate Care Service. A rollout programme is in place to spread the process across all teams working with older people, including the
Accident and Emergency Departments.
The Action Plan sets out how the process will be achieved.
A large number of people have already received the Single Assessment Process and the Kirklees Older Peoples Network have been involved in
evaluating the work with older people of Kirklees to make sure the process meets the needs of older people living in Kirklees.
