Help for you when you leave hospital
The Hospital Enabling Scheme is a service which will help you to get the assessments and support you need when you are leaving hospital. 		Moving on after leaving hospital
When your consultant decides that you do not need to remain in hospital, we will aim to move you on as soon as possible.
