What is the Colne Valley Scheme

The Colne Valley Scheme has been providing a service to adults with varying degrees of learning disabilities, who live in their own homes for 17 years.
The scheme works in partnership with other agencies, such as Green Head Resource unit. The scheme works closely with financial advisors, advocacy services, benefits advisors, and other agencies.
The scheme offers a 365 day service, staff cover weekends and bank holidays should tenants require this service.
However the scheme is not a 24 hour service. At weekends we will provide a limited service, supporting you with your meal or medication. Some people like to relax or spend time with their family or friends and may not need any support other than a phone call. Staff work flexible hours between 7.30 a.m. to 8.30 p.m.
Should problems arise after this time, tenants use the ordinary emergency services (999) or Emergency Duty Services
The scheme offers a service based around tenants' needs, which are identified through their community care assessment
Where is the Colne Valley Scheme based?
The location of the office for the scheme is based in Slaithwaite Town Hall. The offices are on the ground floor, providing easy access to tenants and their family and friends when they choose to call and visit.
To travel by bus from Huddersfield bus station there are many buses that you can catch. There is also a train from Huddersfield which will stop at Slaithwaite station.
Bus numbers are 350, 352, 365, 348. These stop on Manchester road and a short walk is necessary. Buses available every 10 minutes.
348 (via Wilberlee),These buses run twenty minutes to the hour.
390, 391, Colne valley circular, run 15 minutes to the hour.
396, 397 to Slaithwaite at ten past the hour.
What does the Colne Valley scheme do?
As services are based on tenants needs, staff work within times best suited to each individual.
Our staff team includes a manager, three co-ordinators and support staff. We are fortunate to have experienced staff qualified to N.V.Q level 3 and 4. We give new staff a full induction, this includes shadowing experienced staff which enables new staff to be introduced to tenants by familiar faces. We require new staff to complete LDAF with in the first six months of joining the scheme, then progress onto N.V.Q level 3 in promoting independence
What type of support does the Colne Valley Scheme offer?

Staff will give support to tenants
- to maintain the tenancy on their homes meeting the requirements on their individual tenancy agreement.
- within the community, this could be visiting the local shops, hair dressers, barbers, butchers, place of worship, post office, bank and public house.
- who choose to have pets to meet the animals welfare.
- to look for work, paid or voluntary.
- with their hobbies, example, swimming, gym, local public house, restaurant, horse riding, further education.
- with budgets, enabling tenants to manage their weekly income with the knowledge that they are not at risk.
They will also
- offer support with diet, shopping and cooking to enable tenants to live a healthy lifestyle
- give support to home owners to maintain their home to health and safety standards, giving advise and support to enable tenants to live as independently as possible.
- help with health appointments, G.P. hospital, choices, dentist, counselling, chiropodist etc.,
Should the need arise staff would support tenants to obtain the services of an advocate.
Accommodation.
We support people who live in
- private rented accommodation
- people who live in council rented accommodation
- people who are home owners.
These may be in flats, houses or bungalows.
Complaints comments and compliments.
The scheme runs an internal concerns procedure, if this is not enough then support would be given to use the Kirklees complaints, comments and compliments procedure
