Summary of statement of Purpose
Nature of services provided
Home care services are available for people over 18 years old, who have had an assessment of their needs. Support is provided by Home Care Assistants who visit people in their own homes. They help them with personal and household tasks, for example, getting washed and dressed, helping to prepare meals and dealing with bills and letters.
We will provide the following services:
- Community Service – for people who are living in the community and need help to live independently
- Hospital Discharge Service – for up to 6 weeks, for people who are getting better after a stay in hospital
- Rapid Response Service – for up to 5 days, for people who would otherwise be admitted to hospital
- Bi-lingual Service – for people who are South Asian and whose first language is Urdu, Punjabi or Gujerati.
Services may be provided to people who have support needs with one or more of the following:
- physical disability
- mental health (including dementia)
- sensory impairment
- learning disability
- old age
- palliative care
Hours of Operation
Home care services are available throughout the day and evening, every day of the year including Public Holidays. In special circumstances we may be able to arrange night sitting services.
What happens after I have asked for help or support?
We only provide home care services once an assessment of your needs has been carried out. Before your services start, we will carry out a risk assessment to protect the health and safety of you and the staff providing your services. We will then agree with you how we will deliver our services to you. Services will be provided to you by staff that are competent and trained to meet your needs. Once you have started getting your home care service, we will visit you to check that everything is all right – this is called a Quality Visit.
Your needs will be re–assessed at least once a year. If your needs change before this we will arrange for your services to be reviewed.
Aims and Objectives of the service
- to provide a high–quality care service that helps people remain in their own home
- to provide practical support to relatives and friends caring for people in the community
- to enable people to lead as independent a life as possible within their own community
- to provide a service that takes account of people's preferences, wishes, personal circumstances, cultural and religious beliefs wherever possible
- to provide services in an anti-discriminatory way.
How will we achieve these aims and objectives?
- by working to a plan of care that will have been agreed with you
- by treating you and your relatives with dignity and respect always
- by working with you to keep your independence and where possible, help to improve it
- by encouraging you to say what you think about the service
- by ensuring that trained and competent staff provide your services.
