Home care services

Home care services are available for people over 18 years old, who have had an assessment of their needs. Home care assistants who visit people in their own homes provide them with support. They help them with personal and household jobs, for example, getting washed and dressed, helping to prepare meals and dealing with bills and letters.

They can provide the following services:

  • Community Service – for people who are living in the community and need help to live independently.
  • Hospital Discharge Service – for up to 6 weeks, for people who are getting better after a stay in hospital. Rapid Response Service – for up to 5 days, to prevent hospital admission or inappropriate admission to residential or nursing care.
  • Bi-lingual Service – for people who are South Asian and whose first language is Urdu, Punjabi or Gujerati.
Services may be provided to people who need support because of a physical disability, learning disability, sensory impairment, mental health need (including dementia), old age or because they need palliative care.
