When someone asks for help or support, Adult Services carry out an assessment. They can only provide services once we have assessed your need for them. Once we have decided that we can help, a ‘care plan’ is drawn up. This shows you what services you will get and when.
Who is eligible for help?
Not everyone will be able to receive help from Adult Services. They provide services for people with critical or substantial needs. To find out if you can, contact your local Gateway to Care or pick up a leaflet called 'Are you entitled to help from Kirklees adult social care services?'
Even if you are not eligible to receive services from Adult Services they may be able to sign post you to receive help.
With the help of Neighbourhood Renewal funding Adult Services have been able to appoint 7 Gateway Workers as part of a pilot programme in social care delivery. The Gateway Workers are new and can help you get the health and social care advice, information and support you need either from health and council services or voluntary and community groups.
They support:
Older people
People affected by HIV and Aids
People with learning disabilities
People with mental health needs
People with physical and sensory disabilities
Carers
Who can Gateway Workers help?
In particular, they can help you if you have low levels of social care needs. They want to do this to prevent your needs from becoming greater. They want to help you stay independent and active in your community longer so that you can keep in control of your life.
They can help you by networking with other council services, private providers of social care, local community and voluntaryorganisations who offer support, such as befriending and transporting schemes, leisure activities, lunch clubs and good neighbours.
They do this by introducing you to organisations and activities in your local area.
What will Gateway Workers do?
They want to build your confidence so that you can access activities in your community and feel part of it. They will do this in several ways, for example:
They will work with you and your carer, if you have one, so you can get the support you need and have control over your own life.
They will work with you if you are not eligible for adults social care services, such as those delivered by the council or health services. They will do this by looking at what alternatives are available to you and how best these will fit your needs.
They will also help you discover other activities and support in your area, for example, lunch clubs, mobile libraries, sports clubs, adult learning opportunities and befriending schemes.
One of their workers may be able to come with you to your first activity class or scheme so you feel more confident to attend at the beginning.
Partnerships
The project will enable new ways of working to be developed to improve access to public services and other support available in the community. This includes promoting independence, health and well-being, while reducing people’s feelings of isolation and loneliness.
