About Supplementary Information Forms (SIFs)
If you name any of the following academies, free schools, voluntary aided Catholic or voluntary aided Church of England schools amongst your preferences, you should ensure that you complete the school's Supplementary Information Form (SIF).

The academy trusts or governing bodies of these schools decide admissions. If you do not complete a SIF the governors of the school may consider your application of lower priority, as they will not have all the information they require to assess your application against their admission criteria.

You can download SIFs below. You must return your completed SIF to the school itself.
