What services or support are available to you?
Adaptation to your home An adaptation is a change to your home which allows you or your carer greater safety or convenience getting in, out and about your home. It can include things like ramps, grab rails, level access showers and stair lifts which can help you to keep living independently in your own home. Accessible Homes Referral Form
Carephones A Carephone is a special unit that plugs into your telephone line and can dial for help when you need it. We also have other products such as fall detectors and carbon monoxide detectors.
Disability equipment Various equipment is available for you to borrow or to buy. From grabrails costing a couple of pounds to pavement scooters and fitted stairlifts which cost more.
Home support services Home care services are available for people over 18 years old, who have had an assessment of their needs. Support is provided by Home Care Assistants who visit people in their own homes.
Leaving hospital / care Information which will help you to get the help you need when leaving hospital.
Occupational therapy Occupational Therapy is provided by NHS Kirklees. Occupational Therapy work with people to identity what they need to live independently and safefly.
