Care and support - your way
Putting People First is Kirklees Council's response to the Government's reforms in Adult Social Care. It is about giving anyone who needs adult social care the opportunity to receive the support that they want, when they want it.
Putting People First will give you the opportunity to make decisions about how you can access the support services for your requirements.
You can choose what approach you want to take to access your support requirements.
- Let us work through the process with you.
- You want to have some control over parts of your support but would like us to take control over some areas - the 'pick and mix' approach.
- You take complete control, write your own support plan, search and purchase your own support.

Information plus
Advice and signposting to voluntary groups and other agencies. Preventative services such as equipment, blue badges and screening for eligibility to adult social care if needed. ![]() |

Person led assessment
A means of identifying a person's needs and desired outcomes. The person should be encouraged to complete the forms but support is available if needed. A person is given an indicative budget at this stage. Regain independence This stage includes any services that aim to help people stay safe and regain independence. ![]() |

Support planning
This is led by the person with support if needed. It is a means of planning what support can be put in place to meet someone's outcomes using the indicative budget. It includes creative and cost effective solutions that meet the person's desired outcomes. It also includes sourcing support from community resources and existing services where possible. ![]() |

Putting support in place
The person has choice and control about how support is set up and managed within their budget. They may choose to get up and manage their own support or ask for this to be done for them. Support will be flexible and personalised to meet the persons needs. ![]() |

Review The person should be encouraged to review their own needs, support and outcomes and to revise their own support plan. Support is available if needed. |
Self-directed support
Self-directed support is simple and easy to understand. All it means is supporting people to tell us what is important to them and their families and helping them to plan their support including how to use the money available to them to achieve their personal goals and identified social care outcomes.

Person-led assessment
Where an individual has been assessed as having care and support needs they will complete 'My Life' which is a Person Led Assessment. Points are allocated from the assessment and this is converted into a monetary value. This is called an indicative budget.

Support planning
Following the indicative budget people will then spend time planning how best their needs will be met. This will be written down into a support plan. Once the support plan is agreed including the types of service and support the person thinks is right for them the indicative budget becomes a personal budget.

A personal budget is the amount of council money given to fund a person's care and support. Any funding given has to be in-line with an agreed support plan.
Personal budgets come in the following formats:
- a direct (cash) payment.
- by way of an 'account' held and managed by the Council as requested by the person.
- a mixture of the above.
- an "Individual Service Fund" where a third party holds the budget on behalf of the person.
This kind of support will probably change the demand for more traditional care services. People will have more flexibility and be able to choose what suits them best.
