Help for you when you leave hospital (Hospital Enabling Scheme)

We can help if:
  • Your are anxious about being able to cope at home.
  • You are worried about whether your family and friends are going to be able to support you when you go home.
  • You feel you have lost the confidence to live independently.
  • You feel you would benefit form having someone come into your home at certain times to help with washing, dressing, making meals, shopping, laundry or other tasks until you have fully recovered.
  • You have been receiving rehabilitation and feel it would be useful for this to continue when you go home.
  • You need help to regain your independence.
What you need to do

Ask to be visited on the ward by a social worker.  They will assess your needs and discuss with you the kinds of support which may be useful when you go home.

If we all agree that support is necessary, we will then write a care plan which sets out the care you will receive at home.

An occupational therapist will also assess your needs and offer advice on how we may be able to help you improve or maintain your independence at home.

When you go home, our home care support workers will visit you at agreed times and help you to regain your independence as you recover.  This support can last for up to six weeks.  If you still need support after six weeks we will then arrange for another service to provide support.

About the team

The Home Care Enabling Team is made up of the following people:
  • Social worker - who visits you on the ward to assess your needs.
  • Home care manager - who plans and organises the agreed support when you are discharged from hospital.
  • Occupational therapist - who assess your ability to undertake daily tasks at home. This person gives advice on how you can maintain and improve your independence.
  • Home care support workers - who will help you to regain your independence by supporting you in carrying out day-to-day tasks.
