When you are in care you have a care plan and this needs to be talked about every few months - this is called your review. Every time you have a review you need to tell people looking after you how you are doing. You can do this by filling in one of these online forms.

After you've applied

2 weeks after your review you should get a letter with all the important things talked about in your review to help you remember what was said and what's going to happen next. If you don't get this you need to ask your social worker for it.
