Leaving care
When a young adult leaves care at any time after their sixteenth birthday they get help from the Children and Young People Service Leaving Care Team until at least their twenty-first birthday. The team works with looked after young people (in care) in their placement from the age of 15 and half to start to prepare you for thinking about leaving care.
Every young person who has been looked after for at least 13 weeks and leaves care will get a personal adviser from the Leaving Care Team to provide advice, including practical advice and support. This could be;
- helping you to budget
- making sure you have somewhere to live when you leave care
- helping you get to appointments
- supporting you to learn to live on your own
The team will work together with you to make a plan which is called a Pathway plan which sets out things like where you might want to live and how you are going to keep in touch with your family if you want to.
